For a referral to the Age Activity Centre,
simply contact your Social Worker or Health Carer,
and ask them to get in contact with us. Once we have been
contacted -we will need to make a special appointment to
interview you at home.
We may also require from your Social Worker/Carer any relevant case
histories or background information that will be useful in
helping us to assess your needs. Once we have interviewed you,
your case will be processed and all relevant information will be
reviewed by our Managing Director, who will then decide whether
or not becoming a member of the Age Activity Centre will be the
best option for everyone concerned.

your interview with us is strictly confidential and will not be
released without your written permission.
